Rush no more than 4, have a spy on him, make him hold the ball longer with a blanket pass coverage, that way he will be forced to move his feet and use that knee/ankle more than he wants to. It will grind him down. If you blitz him, he will see it and you just made a quick decision for him to get the ball out of his hands fast and on to the next down.
