And this is what the vast majority of the plays where he ends up on the ground look like.

I agree that it is a physical issue.  You can tell when he picks up that Right foot to make his kick step, he kind of jerks as the hip isn't moving the rest of the leg out like he wanted it to.  Then, he's beat and trying to play catch-up.
