Well, the term "opt in" is used often when referring to it. So I assume the medicare for all tax would be broken out of your federal taxes and only applied if you opted in.

Similarly to how social security, medicare and state and local taxes are separate from your federal taxes.

If this isn't the case, then I'd amend my opinion, but this is the impression I've gotten during the discussions.

Your surprise about this makes me wonder if I'm misunderstanding the proposal (and universal healthcare in general), but to my knowledge, federal taxes don't go towards the ACA either. As far as I know, the ACA is funded by those who use it and those who paid the penalty (when it was legal) for not having insurance.

I'm open to sources from anyone who knows more definitively.
