I would defintely put a tag on him, I just don't know which one I would use.  I can make an argument for and against each one, but really can't decide which is the best option.

Lou is the huge majority of the problem. Emphasizing fundamentals like tackling is 100% a coach responsibility. Not adjusting the scheme to fit what his players do well is 100% a coaching flaw. Playing struggling veterans over the kids is 100% a coaching decision.
